چکیده

doi:10.1016/j.jtcvs.2008.01.029 gical intervention, the risk of prosthetic endocarditis after valve replacement is very high in this group of noncompliant patients. Excessive tissue damage and annular abscess, on the other hand, may prevent simple conventional repair. I refer to the procedure as ‘reconstruction’ as opposed to ‘repair’, when 1/3 or more of the valve has to be removed. Reconstruction of the MV, as described here, could be a valuable alternative under these circumstances.

Management for Active Infective Endocarditis with extensive aortic root abscess

Results Our surgical strategy for active IE is 1) complete debridement of infective tissue, 2) reconstruction with biological material. Six cases of 21 required reconstruction of the aorto-mitral continuity using butterfly shaped bovine pericardial patch in 4, and mono bloc aorta-mitral tissue valves replacement in 2. For the other 15 cases, resected aortic annulus were reconstructed using peri...

Images in cardiovascular medicine. Acute mitral regurgitation due to a torn porcine bioprosthetic cusp.

A46-year-old man presented with a 48-hour history of fevers, sweats, and dyspnea. He had bacterial endocarditis of the native mitral valve 12 years earlier with subsequent mitral valve replacement (MVR) with a Hancock II porcine valve (Medtronic Inc). Examination revealed a Grade 3/6 holosystolic murmur at the apex radiating into the axilla with crackles heard halfway up both lung fields. A dia...
